Does anyone have any recommendations as to what should/shouldn't be in toolkits to be used with Cat 3 AND Cat 4 material? I want to make sure I haven't missed anything.

The standard stuff:
Fire extinguisher(s)
PPE for all personnel
first aid kit
flares/portfires (spares) and handles if you use them
caution/barricade tape
rangefinder is handy for measuring safety distances
shovel(s)
plastic sheet in case of rain
hammers
stakes
flashlights (torch?)
headlamps
rakes for cleanup
garbage bags
pyro gear:
quickmatch,visco,delay chains, standard chains
tape (masking, electrical,duct)
utility knives
anvil cutters for fuse
ematches, wire strippers, scab wire
hope that helps a bit.

plastic sheet in case of rain
I recall, that in case of rain and bad weather, each mortar/candle/etc. mouth should be covered with a piece of aluminim paper and tied with a string.
Why we didnt use plastic, i dont know? Maybe because of the chance of static electrisity. The humidity of rainy weather seems to strengthen chance of the spark you dont want

Silver tape is great for most things. Waterproof, spark-proof, can use it for fusing, and good 'witness tape'.
